Branch 2017-2018 School Year Registration

2017-2018 Branch Registration Info.

 Currently registering renewing 2017 summer members and new membership!  You must register at the branch your child will attend for the 2017-2018 School Year. Please contact the Branch for more information on registration times, parent orientation and any other inquiries.  

 

*A child must be in school and between the ages of 6 through 18 years of age to attend a Branch location.

 

 

At each location, we have a set amount of members in which we register. Depending on the location, it can fill very quickly!

 

 

*Once all spots are filled at a Branch, a waiting list will be started for the duration of the current school year.

 

 Membership Renewals are for  current summer members only. A non- refundable $10 per child renewal fee will need to be paid upon completting renewal form. Form of payment accepted will be credit/debit card or money order only. Checks will not be accepted.* Please see Kromer page for more info on additonal Activity Fees due upon registration.

 

New Memberships are for anyone who is a new member and will attend the 2017-201 School Year. The membership fee of $35 and $10 Activity Fee will be due upon registration in the amount of  $45 per child. This is a non-refundable fee. Additonal Activity Fees may apply.    Form of payment accepted will be credit/debit card or money order only. Required Information & Documentation for Branches:

 

Member information- including but not limited to name of child, DOB, school name, grade, school id #, etc.

 

Parent/ Guardian Information- including but not limited to name, address, and contact Information, etc.

 

Household Information- including but not limited to annual income, government assistance, ethnicity, etc.

 

Individuals to be listed as Emergency Contact/Authorized Pick Up - For emergency contacts, an address, phone number and relationship type must be provided. For individuals authorized to pick up, a phone number and relationship type must be provided.

 

Be able to provide any medical Information including allergies, medical conditions, or medications taken. *Make sure all TB and immunizations are up to date prior to enrollment.

 

Doctor Information -name of physician, address, and phone number.

 

Branches must provide medical insurance information.
